In June 2024, 17-year-old Axel Rudakubana killed three children—Bebe King, six; Elsie Dot Stancombe, seven; and Alice da Silva Aguiar, nine—at a Taylor Swift-themed dance class. A public inquiry has since revealed “catastrophic” shortcomings in parental oversight and local authority responses. The investigation highlighted a repetitive cycle of referrals, evaluations, and transfers between agencies, which did not prevent the “predictable and preventable” tragedy.
“We are prepared to reveal key individuals from five agencies unless they face adequate disciplinary action,” said Chris Walker, the legal representative for the families. “Their behavior has been unacceptable, and their failings are entirely unacceptable.”
Agencies Under Scrutiny
Walker specifically mentioned Lancashire Police, Lancashire Social Services, CAMHS (Child and Adolescent Mental Health Service), FCAMHS (Forensic Child and Adolescent Mental Health Service), and the Prevent initiative. The latter, a Home Office program designed to counter extremism, was criticized for its lack of ideological clarity. “Prevent has struggled to grasp the concept of ‘ideology,’ which is central to preventing violent intent,” Walker added.
“The families are deeply unsettled,” Walker stated. “Each new report or incident brings the horror back, as if they’re reliving a movie.”
Call for Accountability
Following the 760-page Phase One report, the Southport Inquiry will enter Phase Two in the coming weeks, focusing on how effectively systems identify and manage risks linked to extreme violence. Walker emphasized that merely having recommendations in a report is insufficient. “We cannot afford to move from one disaster to another without meaningful change,” he said.
“No agency or multi-agency framework took ownership of the grave risk posed by the attacker,” Walker noted. “His autism was used as an excuse, masking the true danger he represented.”
Walker has contacted the involved entities and will push for naming individuals if disciplinary actions are not completed. “Disciplinary proceedings were demanded months ago, and now the report is finalized, we will ensure they are resolved to our satisfaction,” he declared.
